Telehealth patients must meet the following conditions: Live more than 35 miles away, must be over 18 years old, and must have the following available at the time of the appointment: A device with video capability, a blood pressure monitor, and a body weight scale. You must be logged in 5 minutes before your appointment to ensure proper connection.
